Understanding the real problem

The project needed to connect community-based health checks with clinical oversight. Field workers required a practical mobile workflow, while nurses needed consistent records, visit visibility and timely alerts without relying on fragmented manual communication.

Designed around people and operations

i2 translated the research and service workflow into connected mobile and web experiences. Visit scheduling, GPS-supported tracking, health records, Omaha System scoring, reminders and escalation paths were designed around the work of field staff and nurses.
